Prescription Ordering

A minimum of three clear working days is required to process all repeat prescription requests. ‘Working days’ are classed as Monday to Friday (excluding all Bank Holidays). 

So for example, if you would like your medication to be ready on a (non-Bank Holiday) Monday afternoon, you will need to order it on Wednesday morning at the latest.

New Patient Registration

Registration documentation MUST be completed and submitted to us as soon as possible, allowing us time to enter your details onto our clinical system and for your notes to arrive from your previous Health Centre.  Please do not leave it until the day you need to be seen at the Health Centre.  (We will of course make exceptions where there is an urgent medical need).  

To register with the practice please click the appropriate link at the bottom of this page to access the registration form for completion. One form needs to be completed for each person registering. This can either be filled in online or printed and brought in to the surgery. Forms filled in online will need to be signed at the surgery.We will require you to bring in one form of ID for each patient who is registering. 

Alternatively you can come into the surgery and we will provide you with the relevant forms to be completed.

You will also be asked to complete a health questionnaire.

Suitable forms of ID:

A newer style driving licence (with your photo printed on it)

OR

A valid passport 

OR

A utility bill with the address you provide us with on your registration form

All new patients are invited to have a health check. This health check, along with a questionnaire, provides us with an opportunity to meet you and obtain any important background medical history before your medical records arrive from your former practice.  

Please remember to bring any health related documents with you when you attend your first appointment, along with details of any medication that you may be taking.
