Registering with the surgery

Tamar Valley Health is one GP Practice but has 2 sites, one in Callington and one in Gunnislake. You should register at the Practice that is closest to your home address:

Registration documentation should be completed and submitted to us as soon as possible, allowing us time to enter your details onto our clinical system. Please do not leave it until the day you need to be seen at the Health Centre. (We will of course make exceptions where there is an urgent medical need).

Alternatively you can come into the surgery and we will make arrangements to help your register.

Change your personal details

Please let the practice know if you change your personal details.

Please note that if you have changed your surname, we will need proof of identification with your new surname, for example, a marriage certificate.

Temporary registration

If you are ill while away from home or if you are not registered with a doctor but need to see someone, you can receive emergency treatment from the local GP practice.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP.

To register as a temporary patient simply contact the local practice.

Blood and organ donation

We will no longer record your blood and organ donation preferences in the practice. If you wish to do this, you can do so directly through the blood and organ donation online registration websites, NHS account/ app or by phone:
