Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number. Your NHS number is also available on the NHS account/ app and on your prescription.

Newborn babies must be registered with an NHS number.

Registration documentation must be completed and submitted to us as soon as possible, allowing us time to enter your details onto our clinical system and for your notes to arrive from your previous health centre. Please do not leave it until the day you need to be seen at the health centre. (We will of course make exceptions where there is an urgent medical need).

Forms filled in online will need to be signed at the surgery. We request that you bring in one form of ID for each patient who is registering where possible.

Alternatively you can come into the surgery and we will provide you with the relevant forms to be completed. You will also be asked to complete a health questionnaire.

Where you are registering a child please bring their red book with you to the surgery so copies of their immunisation history can be copied and scanned to their records.

Suitable forms of ID

  • A newer style driving licence (with your photo printed on it)
  • A valid passport
  • A utility bill with the address you provide us with on your registration form

Change your personal details

Please let the practice know if you change your personal details.

Please note that if you have changed your surname, we will need proof of identification with your new surname, for example, a marriage certificate.

Temporary registration

If you are ill while away from home or if you are not registered with a doctor but need to see someone, you can receive emergency treatment from the local GP practice.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP.

To register as a temporary patient simply contact the local practice.

Blood and organ donation
